I understand that I will have to sign a release form before every training service received. I understand that I will have to provide feedback on the training service received. I agree to arrive on time for any training appointments and to provide at least 24 hours notice in the event that I need to cancel or reschedule my appointment. I understand that failure to provide adequate notice or no-show for my training appointment will result in elimination from the training program and ineligibility to participate in any future trainings at Haven. I understand that all services will be performed by licensed technicians in a training environment which may result in multiple observers and/or technicians in the room during the service. If I am receiving a training waxing service, I attest that I am not taking any medications topically or orally that contain retinol such as Retin-A, Differin or Accutane. I understand that in order to receive a training waxing service, I must allow 4 weeks of hair growth before the waxing training appointment. I will be contacted 4 weeks prior to a training appointment whenever possible to allow time to prepare. Occasionally, I will also be contacted on shorter notice and will be booked if I meet the criteria at the time. I will not be penalized if I do not meet the criteria or am unavailable for a training service. I understand that waxing during the menstrual cycle is possible but is often more painful and reactive than other times. No facial waxing can be performed on the same day as a facial service. No body waxing can be performed on the same day as a body treatment. I agree to arrive for my training appointment clean. I understand no training massage or body treatment may be performed during pregnancy.
